Violations of Controlled Airspace Survey

Background

Airservices Australia is undertaking an ongoing review of air safety occurrences related to Violations of Controlled Airspace (VCA).

One problem with violations of controlled airspace reports is that in nearly all cases they are generated by Air Traffic Control (ATC) and commonly fail to capture the human factor issues that may have triggered the incident. It is the view of Airservices Australia that to improve safety standards and help reduce violations of controlled airspace, we need to develop good feedback links. These would provide reliable information of how the flight crew perceived the incident.  With this data, collectively we will be better able to determine whether any changes are required to procedures, documentation, training or facilities.

The review will involve all flight crew involved in ATC reported violations of controlled airspace. Routine notification of such safety occurrences from Airservices will alert us to those instances which need to be part of the review.

The survey should take no more than 10 minutes to complete. Your assistance in helping to promote safety is greatly appreciated.

The report does not include any personal details which would allow any external company or agency involved in the study to identify you.  As a consequence, your confidentiality is assured.

The review team will be looking for trends, trigger events and issues rather than individual performances.

Conclusion

Airservices Management has endorsed the review given that it aims to improve the safety and efficiency of the ATS system. Your participation would be greatly beneficial.

The results of the review will be published in appropriate industry safety newsletters.  So please spare a few minutes.
